1. Your ingame name:
2. The offender's ingame name:
3. A description of what rule they broke/how they broke it:
4. Evidence/screenshots.

That doesn't mean it's okay, if you don't do the correct format, your report will prob be ignored, just saying.

 

The report does not contain enough evidence for the staff to issue punishment to the player.
