I recommend looking at accepted Mod applications. You gotta add way more still. Add colors and images to make it prettier and not scrunched together, and if you want add some nifty symbols in the title. Sometimes it also looks kinda better if you center it. (Where it says like B I U A A a/a whatever, look for the line graph and click center.)
