Why do you think you should become a mod? Because I have been mod on many server's, and I have played on mineverse in 5 years. And I have reported and getted players banned, for hacking.
How long can you be active on the server everyday? I can be activete every day
How long have you been playing Mineverse? I have played on mineverse in 5 years
Have you ever been banned in the past? (Forums & In-game) I have never been banned before.
Do you have any past experience as a moderator? Yeh, I have been Owner,Dev,Co-Owner,Admin and Mod on many server's + I have setted up 3-6 server's

In order to become mod, you need to have 50 messages on the forums (you have 8), and Skype/TeamSpeak which you didn't answer.
