For the last question- They only asked if you had past experience with being a mod. The answer is either yes or no, maybe a little detail, but not three stories in one. Then, after that, you go on describing yourself and reasons why you should be a mod. All of that should be in the "Why you think you should become a mod" question, not the experiences w/ mod question. Personally, I wouldn't like reading a whole paragraph on a story about how you didn't grief. But this is your application, so I'll leave it at that.
