I read your previous application and I didn't like it, it had very bad grammar errors, immature comments/replies and this Application isn't good either.
You have to follow the template, you cannot put "Anonymous" they need to know, otherwise they wouldn't be asking.

 

This is a big improvement from your last app, but it's still not quite there.
