Rank premium missing

Hi I purchased a premium rank a month ago I changed my name on march 06 and now I don't have it anymore. Can you please help me get it back. My old ign was dontrunfrommekid and is now itakekids.

The purchaser details used to make the donation or any proof of the donation.
