Use this form to report a player for breaking any rule. Please include:

1. Your forums account name: wizard934
2. The offender's forums account name: No one broke any rules
3. A description of what rule they broke/how they broke it: My friend UltraTitan can't create his own forums account
4. Evidence/screenshots. It comes up with an error page if you guys need a screenshot i can provide one but for now I'm just going to leave it at this
