Some more damage done more recently:

 

There was like 5 mins where all plugins messed up and anyone could break any blocks

 

Some of the best Hacked Clients offer a very good 'Nuker' and you could bypass any protection plugin.

 

I figured it's due to people breaking blocks right before the server crashed, which happens a lot, and the blocks didn't automatically reappear.

A mod can close this now, as Cyp was online and fixed everything.
