Well this is quite simple, for screenshots an easy way to do it is open your resource pack folder and then click on your Minecraft folder. You can then click on screenshots witch will load all of your in-game screenshots, from there just drag your screenshot to the report form For video all you have to do is upload them to youtube and then leave a link to the youtube video
