Ok before I start this off, im not discrediting your style. However, there is a more simply way that might make the lives of the people a whole lot easier. If you try a server called <edit> and make a appeal or apply or report a form comes up, not a google forms. a customly made form with questions and boxes and the ability to add on at the end for like Mod apps. Here is an example.
